How to Get a Medical Marijuana Card in Queens, NY

1. Register on the NY Medical Marijuana Site

Got to https://padohmmp.custhelp.com/app/adult-patient-registration.  You must have a valid NY driver’s license or state ID and an email address to register.  You need to enter exact information as how it appears on your IG (change of address card is applicable) to the state registry.

2. Evaluation with our Doctor

Book online or call us at 484-320-6550 to set up your appointment to be evaluated by our expert medical marijuana doctor.  All appointments are done via Telemedicine (virtual visit/video call).  You must prepare medical records with diagnosis ready for the appointment.  If you need us to request records from your doctor, please fill out the release form that our staff will send to you.

3. Pay for your Card

Once you are certified, you can pay the state for your medical marijuana card.  Make sure the address on your profile is where you want to receive your card.  Once you pay the state, you’ll receive your card by mail within 7-10 days.  Patients may designate a caregiver.

4.  Visit Dispensary

You may visit any dispensary in the state to purchase medical marijuana once you receive your card.  Make an appointment with the dispensary on your first visit.  Bring your medical marijuana ID card when you visit the dispensaries.
