How It Works
Here’s our easy step-by-step process to getting your med card.
Step 1: Make An Appointment
Call us or schedule online. Booking your appointment is quick & easy. Click here to make an appointment online or call (484) 320-6550 to book an appointment now.
Step 2: Get Evaluated
You will have a 10-15 minute telephone conversation with our experienced Doctor who will review your medical records.
Step 3: Get Your Card
At the end of the consultation, you will receive an email from the state that you are now certified and may proceed in ordering your card.
Qualifying Conditions
Participation in the Mississippi Medical Program is open to individuals with the following conditions or those undergoing treatments for them:
- Acquired immune deficiency syndrome (AIDS)
- Agitation of dementia
- Alzheimer’s disease
- Amyotrophic lateral sclerosis (ALS)
- Autism
- Cancer
- Crohn’s disease
- Diabetic/peripheral neuropathy
- Glaucoma
- Hepatitis
- Huntington’s disease
- Muscular dystrophy
- Pain refractory to appropriate opioid management
- Parkinson’s disease
- Post-traumatic stress disorder (PTSD)
- Sickle-cell anemia
- Spastic quadriplegia
- Spinal cord disease or severe injury
- Ulcerative colitis
Also qualifying is a chronic terminal or debilitating disease or medical condition or its treatment that produces one or more of the following:
- Cachexia or wasting syndrome
- Chronic pain
- Severe or intractable nausea
- Seizures
- Severe and persistent muscle spasms including, but not limited to, those characteristic of multiple sclerosis
